Telehealth Terms and Consent
Effective September 26, 2026
Medical evaluation and consent
You confirm that you are the patient requesting care. The medical intake will be used by a board-certified physician licensed in your state to evaluate your concern and make an individual treatment plan. You agree to provide accurate and complete information and to identify the reason for requesting treatment.
No guarantee
There is no guarantee that a clinician will treat you or issue a prescription. A clinician may approve, change, or decline a requested treatment when it is not medically or ethically appropriate or when misuse is suspected.
Clinical communications
You consent to clinically appropriate communication by secure messages, direct text messages, email, phone calls, video or audio files, and invitations to HIPAA-compliant audio or video services. Secure two-way communication with the clinician remains available during and after the initial evaluation.
Insurance and government payors
Telehealth services offered through this program are cash-pay services. You agree not to submit claims for these services to Medicare, Medicaid, another federal or state payor, or a private insurer.
Pharmacy choice
We may identify a preferred pharmacy for fulfillment, but you may request that an approved prescription be directed to another pharmacy of your choosing when permitted by law and clinically appropriate. Contact support before fulfillment to request a pharmacy change.
Alternatives and emergencies
You may seek in-person care from your primary-care clinician or an appropriate specialist instead of using this telehealth program. If you do not have a primary-care clinician, search for a primary care doctor near you or contact your health plan. For an urgent or emergency condition, call 911 or seek immediate in-person care.
California physician complaints
California patients may submit a complaint about physician care to the Medical Board of California’s Central Complaint Unit through its complaint page, at 1-800-633-2322, or at 916-263-2382.
MD Integrations
To the fullest extent permitted by law, you agree to indemnify and hold harmless MD Integrations, its employees, and clinicians who contract with MD Integrations from claims arising from your misuse of the services, violation of these terms, or submission of inaccurate or incomplete information. This provision does not waive rights that cannot legally be waived.
